Community Recovery Specialist - Per Diem

This is a per diem/Sub position. Hours/Shifts may vary.

Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service.  The programs of Community Care Network serve more than 3,000 people each year throughout Rutland County. The dedicated 350 plus employees of CCN are focused on improving the health and well-being of Rutland County residents and their families living with mental illness, developmental disabilities and substance use disorders.

About The Role:

We have an opportunity currently available for a Community Recovery Specialist to provide support services to consumers living with mental illness, residing in an independent housing program. Support services include coordinating client case management services, planning and carrying out goals of individual's treatment plan, goal setting, independent living skills training and mental health stability. The Community Recovery Specialist will also provide transportation of day-to-day appointments as needed.

What You Need to Succeed:

  • High School diploma.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire.
  • General knowledge of community resources, standard models of service coordination, and/or experience working in a variety of human services settings.
  • Proficient with Microsoft Word and Outlook required.
  • Must demonstrate superior listening, communication and writing skills.
