Engagement Specialist


Community Care Network is looking for an Engagement Specialist to join our team!

About the Role:

The Engagement Specialist plays a critical role in improving client participation in Behavioral Health services by conducting outreach, reducing barriers to care, and ensuring timely follow-ups. This position supports clients through engagement strategies, coordination of services, and collaboration with providers, families, and community partners. They are responsible for re-engaging clients and families who struggle to attend appointments or meetings, such as therapy sessions, case management visits, or treatment team meetings. The Engagement Specialist may also participate in staff meetings, hospital visits, and community events to support clients and educate others about Rutland Mental Health Services.

Responsibilities:

  • Monitor missed appointments
  • Contact clients and families to determine the reasons for missed appointments, collaborate on solutions to prevent future absences, and document all discussions and actions in progress notes.
  • Notify providers when appropriate to ensure follow-up on agreed-upon remedies to improve client engagement.
  • Schedule appointment reminders and follow-ups
  • Work with therapists and case managers to schedule timely follow-up appointments for clients being discharged from hospitals.
  • Assist with complex referrals from impatient facilities by engaging clients directly at the hospital, identifying potential barriers to treatment, and facilitating their transition to outpatient care.
  • In cases of complex client situations or treatment dropouts, engage clients in the community. When requested by external community partners, assess barriers and encourage re-engagement in treatment.

Qualifications:

  • Bachelor's degree in social work, psychology, counseling, or a related field (preferred) OR equivalent experience in behavioral health, peer support, or case management.
  • Experience working with individuals experiencing mental health challenges, substance use disorders, or housing instability.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport.
  • Knowledge of local behavioral health resources and social services.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Proficiency in Microsoft Office and electronic health record (EHR) systems.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
