Community Care Network is looking for a Per Diem Transportation Support Specialist to join our team!

About the Role:

The Per Diem Transportation Support Specialist (TSS) is responsible for providing transportation to individuals, as identified in their ISA and Career Support Plan. This includes supporting someone to arrive at their employment site and home from the employment site, safely. The Per Diem TSS may also provide minimal amounts of pre and post shift supports as needed to some individuals.

Principal Responsibilities:

• Provides transportation to individuals so they can maintain employment and work as independently as possible.
• Interacts with individuals served with respect and in a professional and dignified manner.
• Ensures the safety and wellbeing of individuals served.
• Documents required paperwork (i.e. daily data, service logs, timesheets, mileage sheet…etc.) in a timely manner.

Qualifications:

• High School Diploma or equivalent (GED).
• Must be available Tuesdays and Thursdays.
• Previous experience in developmental disabilities preferred.
• Able to flex schedule as needed.
• Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
• *Successful completion of a criminal history background check after hire.
