Community Care Network is looking for a School Based Case Manager to join our team!

About the Role:

The School Based Case Manager promotes pro-social behaviors and social skills to students within a school-based setting. Provides support to the student to help them achieve academic success. Assist students and families in obtaining recommended service coordination and community support.

Responsibilities:

• Provide evaluation, treatment, consultation, community education and training activities in the school and community.
• Involves transportation of self and client in the provision of services in a personal vehicle.
• Facilitate access and referral to guardians and conservators, self-help groups, advocacy services, and legal advocacy service when available and necessary.
• Timely completion of clinical documentation per agency policy to include treatment plan document, clinical contact notes, time sheets, managed care authorizations, discharge summary and all others as needed.
• Accept cases as assigned by supervisor.
• Attend clinical supervision, and team, department and agency staff meetings as provided.

Qualifications:

• Bachelor's degree in Social Work, Counseling or related field.
• Previous experience assisting with service coordination and supports of students and families.
• Proficiency with computers, iPads and other related technologies and experience entering data into electronic medical records (EMR) program.
• Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
• *Successful completion of a criminal history background check after hire.
