Network is seeking an Advanced Practice Registered Nurse who will provide medical
oversight to a new Certified Community Behavioral Health Clinic project in
Rutland, Vermont.

About the Role:
The
Advanced Practice Registered Nurse works with a team of professional social
workers, clinicians, case managers and others to ensure integrated care for
adults experiencing substance use disorder and behavioral health concerns. The
APRN will provide medications for opioid use disorder (MOUD) for Opioid Use
Disorder (OUD) and conduct health screenings as part of a system of integrated
care and establish therapeutic relationships with clients to facilitate
progress with treatment goals. The APRN will have the demonstrated ability to
balance independent clinical decision making with appropriate consultation from
providers. X-waiver for prescribing buprenorphine required.
Principal
Responsibilities:
- Provide
medication evaluation and management, medical consultation with ongoing
monitoring of effects of medication use in coordination with primary care
providers
- Conduct
clinical evaluations and physical health screenings. Refer for follow up care
as needed.
- Provide
MOUD services; prescribe medications for the treatment of Opioid Use Disorder
(OUD)
- Take
a lead role on an interdisciplinary treatment team, ensuring a coordinated and
responsive person-centered treatment plan and evaluation process.
- Diagnose medical conditions and illnesses
- Document
medical and psychological histories, physical assessment results, diagnoses,
treatment plans, prescriptions, notes or outcomes in Electronic Medical Record
within assigned timeline.
- Develop
and promote contacts, working relationships, and cooperative arrangements
between the Agency and other organizations. This is an essential component of
Vermont's Hub and Spoke model.
Qualifications and
Competencies:
- Master's
Degree in Nursing
- Possess
an unrestricted, current and complete State of Vermont APRN license (or ability
to attain upon employment), in addition to an X-waiver for the prescription of
buprenorphine.
- Demonstrate
current knowledge of psychiatric medications and substance use treatment
practices.
- Experience
working with adults with mental illness, substance use disorders, developmental
disabilities, and co-occurring medical conditions required.
- Solid
computer skills and familiarity with operating an Electronic Medical Record
required.
- Ability
to demonstrate compassion, sensitivity, and reasoning skills.
- Must
have a valid driver's license, reliable transportation with personal automobile
liability insurance coverage at or above the levels mandated by the State of
Vermont.
- Successful
completion of a criminal history background check after hire.
