Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service. The Community Access Program (CAP) offers a comprehensive array of person-centered supports to children and adults with developmental disabilities and their families. This position is responsible for working with CAP to provide one-on-one supports for individuals.
About The Role:
Opportunities are currently available for Direct Support Professionals to provide one-on-one support to adults with development disabilities in the community, or in homes/apartments. This position assists individuals in a variety of day-to-day tasks, such as but not limited to: transportation, medical management, dietary needs, personal care and more. This position will assist in planning and carrying out individuals programs as well as implementing goals identified by the Individualized Service Agreement (ISA). This job is rewarding and incredibly transformative.
