Outreach Specialist - Apartment

Community Care Network is looking for Apartment Outreach Specialists to join our team!

About the Role:

The Apartments Outreach Specialist provides support and guidance to consumers residing in an independent housing program. Support services include coordinating client case management services, planning and carrying out goals of individual's treatment plan/ISA, goal setting, independent living skills training, supporting individuals' overall stability and general wellbeing. The Outreach Specialist will also provide transportation of day-to-day appointments as needed.  This support and service is to assist individuals to reside independently in the community through support, skill development, and education.  This position allows for interfacing with a variety of individuals with differing needs and approaches. 

We are looking for 1 full time employee to work 11-9 4 days with one weekend day required and 1 part time employee to work 11-9 for 2 days with one weekend day required. 

Principal Responsibilities:

  • Serve as a role model, mentor, advocate and motivator to individuals in order to help prevent relapse and promote long-term stability healthy and adaptive social, emotional, and behavioral functioning.
  • Assist in planning and implementing clients' individual plans of care by coordinating Community Case Management services with Clinician/Case Managers/service coordinator as appropriate.
  • Providing a professional and individualized support system that helps to supervise, direct, inform, and assess current and prospective needs of individual.
  • Provides the necessary support to ensure that all basic needs are met, including, personal care, household management, nutrition, medication management.


  • High School Diploma or GED Required; Bachelor's Degree Preferred
  • Able to work alternative work schedules, evenings and weekends as needed for operations and at the request of management.
  • Ability to navigate computers and necessary software to complete client documentation.
  • Excellent interpersonal communication, problem solving, and conflict resolution skills with the ability to appropriately utilize supervision as needed.
  • Able to work independently, part of a team and exercise mature and professional judgment.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.
