School Based Clinician

Community Care Network is looking for School Based Clinicians to join our team!

About the Role:

The School Based Clinician is responsible for the provision of diagnosis and evaluation, psychotherapy, community supports and service planning and coordination to children in crisis, with severe emotional and behavior disturbances in a school-based setting. The services provided will target to improve social skills, emotional regulation, problem solving and the overall mental health service goals for the child. The clinician will develop and maintain close working relationships with internal and external professional staff to provide coordinated, quality services.

Principal Responsibilities:

  • Provide clinical assessments and interventions for students and families.
  • Provide individual, group and family therapy, based on emotional need, maintaining a full case load, based on mental health standards and ethics.
  • Provide follow-up and implementation of reintegration plans for clients who are returning to school from hospitalizations and/or facility stays; and comprehensive follow up to all clients served.
  • Make home visits to support families. Facilitate communication between agencies on behalf of the client. Attend team meetings to plan and monitor services. Provide case management, collateral contact and social support, as needed.
  • Assists family to locate and appropriately use community resources, agencies, organizations and team members to coordinate services and monitor progress toward treatment goals.
  • Provides consultation services to teachers, community partners, and school personnel, as needed. Consultation can be educational or evaluative. Also provides consultation and support to staff.

Qualifications:

  • Master's Degree in a Human Services Field (Counseling, Psychology, Social Work, etc.)
  • Licensed or License Eligible in the State of Vermont
  • 2-3 years' experience providing mental health services for diverse range of school-aged children living with serious emotional disorders required.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.
