Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service. Through this position, you will be responsible for overseeing the service coordination for individuals with developmental disabilities.

About The Role:

We are looking to fill our Service Coordination role on our Home and Specialized Supports Team. This position is responsible for developing services, monitoring effectiveness, maintaining required documentation and meeting regulatory and quality guidelines. You will work closely alongside a team of people to help provide the person-centered service. Ideal candidate will be committed to positive solution based thinking, dedicated to supporting individuals to lead thriving lives and excited by challenges. We are seeking someone who is willing to help facilitate community inclusion, life skills, positive relationships and self-advocacy.

What You Need to Succeed:

  • Bachelor's Degree in related field required.
  • Previous experience working with individuals with developmental disabilities preferred.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont

  • Successful completion of a criminal history background check after hire.
