Substance Use Disorder Case Manager

Community Care Network is seeking a Substance Use Disorder Case Manager in Rutland, Vermont.

About the Role:

The Substance Use Case Manager is responsible for managing an assigned caseload of clients who are engaged with Adult Substance Use Services through coordination of services, provision of supportive counseling, evaluation and assessment of needs.  The SUD Case Manager will work closely with the Program Manager, SUD Counselors to identify individuals struggling to maintain engagement in services, and aid/support to address barriers.  The SUD Case Manager will collaborate with Community Partners to assist in providing referral and easy access to outpatient SUD services for the community.  The SUD Case Manager shall maintain a close working relationship with all members of the treatment team in order to ensure delivery to clients of comprehensive treatment in an efficient and expedient manner. 

Principal Responsibilities:

  • Participate and lead, as directed, in the screening of new service requests within the clinic through means of and agency approved procedures which ensures that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing agency policies and procedures.
  • Accept newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Complete case management assessment and develop an individualized treatment plan with the client based on that assessment for all clients on caseload. Provide on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the treatment plan in order to help the clients to achieve the stated goals and objectives.
  • Participate in interagency planning and service coordination activities as directed to improve and enhance service continuity and effectiveness for clients to include but not be inclusive of referrals to other interagency programs.
  • Maintain close communication with the consulting psychiatrist, Medical Director and/or nurse for input regarding medication compliance, side effects of medication, and/or medication changes as well as alert required members of the staff of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention.
  • Maintain documentation and recordkeeping using EMR as required by the agency, State, and accrediting bodies.
  • Coordinate Co-Occurring Disorders Program in Adult Behavioral Health as needed.

Qualifications and Competencies:

  • Bachelor's degree in psychology, human services, social work or related field.
    • Or an Associate's Degree and 5years experience working with adults in recovery.
  • Possess 1-2 years combination of mental health work experience and training to perform principle responsibilities.
  • Knowledge with services, systems, and programs available in the community including primary health care, residential treatment, Medication assisted treatment, eligibility criteria and intake processes, generic community resources, and mental health.
  • Working knowledge of substance use, relapse prevention, overdose risk, serious mental illness, mental disabilities, including clinical and developmental issues.
  • Understanding the use of medications in the care or treatment of the population served.
  • Ability to work effectively in an often-conflicting environment that dictates high need of service with limited resource.
  • Superior communication skills, verbally and in writing, and able to maintain confidentiality.
  • Ability to work with team members, maintaining effective inter- and intra-agency working relationships.
  • Ability to work independently, performing position duties under general supervision.
  • Ability to engage and sustain ongoing relationships with individuals receiving services.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire.
