Transition Coordinator

Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service.

About The Role:

We are looking for a Transition Coordinator who is passionate about working alongside of transitioned-aged youth in finding and maintaining employment within their desired goals. Transition Coordinator is responsible for building and maintaining relationships with community partners, namely local high schools and their transition teams, as well as with internal agency post-secondary programs, i.e Project SEARCH and College Steps and driving metrics which support program outcomes.

What You Need to Succeed:

  • Bachelor's degree in related field preferred; equivalent experience may be considered.
  • Previous experience working with individuals with developmental disabilities is required.
  • Previous experience in supportive employments is preferred.
  • Must be able to meet pre-employment drug screening expectations of host site.
  • Previous experience working in school's special education department desirable
  • Valid driver's license, reliable transportation, good driving record, and car insurance.
