Community Care Network is seeking a RN or
LPN to join our Certified Community Behavioral Health Clinic in Rutland,
Vermont.

About the Role:
The
CCBHC Nurse will provide high quality health care services for individuals
receiving treatment services substance use disorder. Services provided at
clinic include medication management/assessment for substance use disorders and
primary care screening.
A
qualified Registered Nurse or Licensed Practical Nurse will offer comprehensive
care management, care coordination, health promotion, and transitional care for
those receiving medication assisted treatment. This role is part of a
professional team that includes a program medical director, social workers,
clinicians, case managers, and others, to ensure integrated care for adults
experiencing substance use disorder and mental health concerns.
Principal
Responsibilities:
- Provides
education and assessments to individuals looking to access MAT services.
- Works
closely with clinical staff to monitor impacts of prescribed medication and
care coordination with pharmacy. This may include refills and prior
authorizations.
- Records
patients vital signs, history, and review of systems.
- Performs
medication reconciliation and obtains VPMS reports for review with the MAT
provider.
- Performs
and results urine drug screening and breathalyzer tests as needed. Communicates
results to treatment team.
- May
perform a variety of routine blood drawing procedures to include venipuncture,
routine immunizations, therapeutic injections.
- Provides
education to patient about program guidelines/expectations and treatment goals.
- Provides
education to patients about MAT medications and signs/symptoms of opiate
withdrawal.
- Enhance
patient health and self-management skills by providing education on tobacco
use/cessation, nutrition, exercise, recovery, wellness, chronic illnesses (HTN,
Diabetes, Obesity, CAD, Chronic Pain, Depression), infectious diseases, and
medications.
Qualifications and
Competencies:
- Licensed by the State of
Vermont as a Registered Nurse or Licensed Practical Nurse
- Uphold current Vermont
RN/LPN Licensure.
- Current BLS
certification or completes within 60 days.
- Documented medical
experience with full knowledge of medications and their effects, patient
education examination, diagnostic and treatment procedures.
- Experience working with
adults with mental illness, substance use disorders, developmental
disabilities, and co-occurring medical conditions required.
- Solid computer skills
and familiarity with operating an Electronic Medical Record required.
- Ability to demonstrate
compassion, sensitivity, and reasoning skills.
- Ability to effectively
communicate with clients, peers, and other professionals.
- Must have a valid
driver's license, reliable transportation with personal automobile liability
insurance coverage at or above the levels mandated by the State of Vermont.

a criminal history background check after hire.
