Crisis Case Manager

Community Care Network is looking for a Crisis Case Manager to join our team!

About the Role:

The Emergency Services Case Manager position is primarily responsible for responding to incoming calls to the Emergency Services crisis line: triage calls, make preliminary assessments, transfer calls to crisis clinicians, disseminate information regarding community resources;  participating in community based responses to in-person crisis needs in partnership with crisis clinicians; and maintaining a small caseload for short term case management supports for clients who recently engaged with emergency services.

Responsibilities:

  • Answer incoming calls to Crisis line.
  • Conduct initial brief mental health assessments and either respond to needs or transfer to clinician.
  • Build relationships and collaborate with mental health providers to ensure seamless referral and services from Emergency Services to longer term mental health services.
  • Provide ongoing information, referrals, linkages, and advocacy for other identified needs where appropriate.
  • Meet with established participants as needed to review, evaluate and support completion of their treatment goals.

Qualifications:

  • Bachelor's degree in psychology, human services, social work or related field required.
  • Possess 1-2 years combination of mental health work experience and training to perform principle responsibilities.
  • Able to work alternative work schedules, which includes working rotating weekends.
  • Knowledge of services, systems, and programs available int the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, and substance use treatment programs.
  • Working knowledge of mental illness and substance use disorders including clinical and developmental issues.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
