Intake Clinician

Community Care Network is seeking an Intake Clinician in Rutland, Vermont.

About the Role:

The Intake Coordinator is responsible for ensuring smooth, efficient, person-centered access to services for incoming clients.  The Intake Coordinator will work in collaboration with all aspects of Adult Services including the Clinical team, Community Programs, and Substance Use team to ensure clients are connected with appropriate services based on client preferences and clinical recommendations.  The Intake Coordinator will be connected with community partners for care continuity, collaboration, and facilitation of future referrals. 

Principal Responsibilities:

  • Provision of initial assessments, diagnostic evaluations to determine program eligibility, treatment recommendations, and case disposition.
  • Collaborate with program managers, clinical, and community support teams to provide client with smooth transition into services.
  • Provide client centered, trauma informed, recovery-oriented care and services to clients.
  • Collaborate with community supports team in completing Clinical Re-assessments for established clients receiving care. 
  • Develop and implement intake workflow and procedure ensure timely access and follow-up.
  • Completion of all recordkeeping and data collection as required by program, State, and accrediting bodies.
  • Complete electronic and paper documentation necessary to record and communicate intake/triage activities as outlined by RMHS documentation policy.
  • Assist in developing, implementing, and evaluating adult intake services and processes.

Qualifications and Competencies:

  • Master's degree in Psychology or Human Services, willingness to be rostered with the state of VT required
  • Licensure as LCMHC, LICSW or related field preferred.
  • Ability to work effectively in an often-conflicting environment that dictates high need of service with limited resource.
  • Ability to model and promote recovery-oriented, trauma-informed, person-centered counseling/support to staff.
  • Excellent interpersonal, organizational, and time management skills
  • Ability to balance autonomy and work within team structure.
  • Proficiency with computers, experience entering data into electronic medical records program
  • Prior experience completing thorough diagnostic evaluations, assessments, and formulating comprehensive treatment recommendations.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire.
