Supervisor of Service Coordination - $1,000 Sign On Bonus

Community Care Network is looking for a Supervisor of Service Coordination to join our team!

About the Role:

The Supervisor of Service Coordination will assist with supervision and ongoing support for staff on the service coordination team.  Supervisor will also ensure completion of necessary documentation and compliance with all regulations and help train/maintain job specific skills for the team.  A supervisor can provide brief periods of in person work to facilitate health and wellness checks/services for people receiving supports.

Principal Responsibilities:

  • Instills a sense of calm, guiding staff in a strength-based manner, always focusing on what is best practice (coach staff into feeling more competent, skilled, and independent-not less so). 
  • Ensures that the interests and preferences of the individuals are honored and respected in the Individual Support Agreement (ISA) in addition to assisting to develop the ISA.
  • Documents contacts, visits, concerns, etc. in Electronic Medical Record and E3.
  • Supports Service Coordinators to meet deadlines effectively.
  • Assists Service Coordinators in development of funding proposals for existing and new individuals assigned.
  • Coaches staff in order to help them meet productivity requirements as defined by Managers.
  • Provides direct clinical supervision to Service Coordinators on a regular basis.

Qualifications:

  • Bachelor's degree in related field preferred; equivalent experience may be considered.
  • Eligible for Qualified Developmental Disabilities Professional (QDDP) status.
  • Previous experience in developmental disabilities preferred.
  • Proficiency with computers, iPads and other related technologies and experience entering data into electronic medical records (EMR) program.
  • Ability to work with others in a positive, responsible manner.
  • Strong writing and organizational skills in order to receive and convey information in efficient and effective manner. 
  • Able to flex schedule as needed.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
