Transition Specialist

Community Care Network is looking for a Transition Specialist to join our team!

About the Role:

The Transition Specialist builds and fosters relationships with community partners (i.e. Rutland Regional Medical Center, law enforcement, schools) around transition needs.  The Transition Specialist conducts training, transition and crisis planning, and step up/step down planning.  In addition, this position assists CCN teams to ensure eligibility status is determined, both clinically and financially. This position is a liaison for new and existing families, individuals and teams until long term supports are engaged and maintained.

Principal Responsibilities:

  • Acts as an interim contact when an eligible individual is waiting for permanent supports to be in place.
  • Develops transition plans and ensures Developmental Service staff are adhering to timeframes and supports are started.
  • Establishes transition plans and placement relationships for children aging out of Department of Children and Families custody so there is a warm, well considered transition.
  • Assists high needs individuals to establish guardianship, payee services and social security prior to adult Developmental services.
  • Assists individuals not receiving Home and Community Based Services to assess career goals and best fits.
  • Acts as a temporary support for individuals moving from Bridge, or other internal CCN teams while they wait for permanent supports to be in place.

Qualifications:

  • Bachelor's degree in related field preferred; equivalent experience may be considered.
  • Previous experience in supportive employments required.
  • Previous experience working with school's special education department desired.
  • Previous work with crisis triage and engaging teams to build supports in step up/step down scenarios.
  • Proficiency with computers, experience entering data into electronic medical records (EMR) program.
  • Ability to work with others in a positive, responsible manner.
  • Strong writing and organizational skills in order to receive and convey information in efficient and effective manner. 
  • Flexibility, dependability, and ability to work independently.
  • Able to flex schedule as needed.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
