Treatment Court Case Manager

Community Care Network is seeking a Treatment Court Case Manager in Rutland, Vermont.

About the Role:

This position will be responsible for working with participants in the Rutland County Treatment Court (RCTC). The Case Manager is under the general direction of the RCTC Oversight Committee and the Manager of Substance Use Disorder. In general, the RCTC Case Manager will follow all cases of involved participants, support participants in following through on their Individual Treatment Plan, attend RCTC Team meetings and communicate both verbally and in writing to the RCTC Team regarding progress and compliance issues.

Principal Responsibilities:

  • Refers RCTC participants for screening, assessment, appropriate Substance Use Disorder (SUD) treatment and ancillary services as outlined in the participants individualized treatment plans.
  • Refers, consults, coordinates and assists RCTC participants with life skills training in such areas as: time management, job searches, budgeting, locating and securing safe and sober housing and pro-social recreation activities.
  • Monitors services received by participants to make sure they are following their treatment plans.
  • Participates fully as a member of the RCTC team with the Treatment Court Judge, State's Attorney, Defense Attorney, the RCTC Coordinator, Law Enforcement, Probation and Parole and any other team members.
  • Acts as liaison between the treatment providers and the RCTC team by participating in staffing's to provide input and information so the team can determine incentives, sanctions, terminations, phase advancement and graduations.
  • Attends RCTC hearings.
  • Provides weekly updates on RCTC participants to the team utilizing the RCTC reporting form.
  • Participates and engages in process and outcome evaluation reviews and evolution of the RCTC design.
  • Coordinates and participates (when necessary) in the collection of urine drug tests and breathalyzers per RCTC guidelines.
  • Accepts and completes additional relevant duties as assigned by supervisor. 

Qualifications and Competencies:

  • Bachelor's degree in related field or equivalent in education and/or experience.
  • Must have a general knowledge of: community resources, treatment courts, the criminal justice system, Medicaid, standard models of service coordination, 12 - step/recovery methodology, relapse and experience working in human service settings.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • Successful completion of a criminal history background check after hire.
