The Early Childhood Case Manager serves children birth to six and their families, providing community supports and service coordination. Services are provided in the office, community, early education settings and homes, or by telehealth. Case managers have excellent verbal and written communication skills; efficiency in documentation and record-keeping; proficiency with online platforms, and strong customer service skills. Involves transportation of clients in personal vehicle. Clean driving record and access to reliable transportation are a must. Bachelor's degree required, BSW preferred.
The Billing Director oversees the billing processes and the daily activities of staff in order to generate accurate billing while ensuring compliance with Federal, State and Third-Party Payer coding rules and regulations.? Duties include maintaining a solid understanding of workflows and procedures necessary to produce the Agency’s billing, along with the selection, scheduling, reviewing and training of the billing staff. The Billing Director partners with Program Directors and the accounting team to ensure all appropriate measures have been taken to account for the Agency’s services. The Billing Director will provide regular updates to Senior Leadership regarding the financial status of billing processes.
Care Coordinators assist people receiving services and the families of children and youth referred to external providers or resources in obtaining an appointment and tracking participation in services to ensure coordination and receipt of supports; also assists people receiving services and families to access benefits, including Medicaid, and enroll in programs or supports that may benefit them. The ideal candidate will be skilled in collaboration, problem-solving, and compassionate communication.
The Behavior Interventionist is responsible for providing mental health services, consultation and behavioral intervention to students with autism or other disorders in the school setting. The Behavioral Interventionist helps students develop social skills, improve their ability to learn in school, and eliminate or reduce negative or disruptive behaviors.

The School Based Case Manager promotes pro-social behaviors and social skills to students within a school-based setting. Provides support to the student to help them achieve academic success. Assist students and families in obtaining recommended service coordination and community support.
The School Based Clinician is responsible for the provision of diagnosis and evaluation, psychotherapy, community supports, service planning, and coordination to all children who are needing additional mental health supports. Those services can include crisis interventions, trauma informed supports and resources, which are embedded directly into the child's school and therefore more easily accessed.
The Inclusion Coordinator is directly responsible for overseeing a coordinated approach to providing inclusion services necessary for Early Head Start and Head Start children to be successful in the classroom. The Inclusion coordinator supports all students to ensure the success of each student in an inclusive classroom environment. Collaborates with teaching a team, classroom supervisors and Education manager to ensure behavior supports are met. Collaborates with Education Manager and local IDEA to support individualized needs being met. Conduct classroom observations and provide classroom teams with individualized support as needed.
The Adult SUD Counselor provides services and supports to adults seeking treatment for substance use and co-occurring disorders. This position works as part of the Adult multi-disciplinary team to ensure services are provided utilizing best practices based on assessed needs, individualized goals, and with client preferences. The Counselor role provides opportunity for growth, creativity, and exploration of professional interests. Clinical services include individual and group psychotherapy, service planning and coordination. This role receives regular individual supervision toward licensure, in addition to weekly group supervision, and training opportunities.
The Adult Clinical Programs Manager provides guidance, instruction, direction, leadership and support to Adult Clinical Program's clinical services including but not limited to supervisors, clinicians, and embedded case managers. Career Support Professionals carry out the services of the Individual Support Agreement and Career Support Plan by identifying individuals’ strengths, abilities, and interests. In addition, this position provides direct support to individuals that is consistent with their employment and career goals who are on their case load or assigned. Career Support Professionals maintain a caseload of individuals. They are responsible for staying in contact with the individuals, parent and/or guardians, and employers in relationship to direct placed employment. 70% of the position will be doing career development and 30% will be doing direct support.
The Level 2 Direct Support Professional is an entry level position for which all training is provided. This position provides direct support to individuals with developmental disabilities so they can maintain employment. The DSP II works with individuals on identified goals emphasizing individuals’ strengths, abilities, and interests.
The Community Outreach Specialist provides support services to consumers residing in an independent housing program and in the community. The support services include coordinating client case management, planning and carrying out goals of individual’s treatment plan/ISA, goal setting, training independent living skills, and supporting individuals’ overall mental health stability and general wellbeing. The Community Outreach Specialist will provide transportation of day-to-day appointments as needed. The Child Outpatient Clinician is responsible for providing clinical services to meet the customer service, quality and overall goals of Rutland Mental Health Services. Meet the program’s uniquely-defined internal/external compliance standards and performance measures. The part time Wellness Wing Outreach Specialist provides support services to consumers through our 12 hours/day 7 days a week drop in center. Support services include; supportive counseling, group education, coping skills, and ADL instruction. This position allows for creativity and developing staff areas of interest in how they provide services and supports to individuals. This program is to provide a place and community for consumers to feel safe, learn skills, connect with peers, and identify new areas of interest.
The Law Enforcement Liaison Clinician will work with core emergency services team and law enforcement to identify clients in need of services and to take a more proactive approach in meeting program demands. The overall goals are to reduce unnecessary Emergency Department utilization, assist in diverting inpatient psychiatric admissions by providing counseling and therapeutic support to clients in crisis, with the emphasis on reducing unnecessary interactions with law enforcement to decrease incarceration rates and over utilization of other Emergency personnel. This position is responsible for providing mental health crisis assessment, intervention and brief treatment services to a diverse population of clients in crisis. In addition, the crisis clinician will provide appropriate recommendations for care and linkage services for clients requiring on-going treatment.
The Psychiatric-Mental Health Nurse Practitioner works with a team of professional social workers, clinicians, case managers and others to ensure integrated care for adults experiencing serious mental illness. As a member of the client care team, the PMHNP will establish therapeutic relationships in order to facilitate client progress with treatment goals. To be effective, the PMHNP will have the demonstrated ability to balance independent clinical decision making with appropriate consultation from providers.
The Intensive Direct Support Professional (IDSP) is responsible for providing the appropriate support and guidance as identified in the Individual Support Agreement (ISA) for dual diagnosis individuals identified for high need supports. The IDSP provides support within the individual’s home environment; prompting, supporting, and encouraging completion of Activities of Daily Living, getting to and from appointments, engagement in social activities, and developing/maintaining independent living skills (i.e. grocery shopping). This position also provides opportunity for preferred community engagement. The IDSP is vigilant in providing quality intensive specialized instruction, behavioral intervention, and support services to promote emotional and behavioral health. The IDSP attends team meetings, completes required documentation and contributes to the development of Behavior Support Plans and Individual Support Agreements.

Community Care Network partners with students to provide internship hours and field instruction necessary to complete their degree. Internships vary depending on a student's level of education and specialty. The intern will work under close supervision of on-site professional in the agency. Intern will receive ongoing guidance and support from off-site educator who supervisors their field placement.
This is a per diem position (2-16 hours per week depending on case load) with a lot of flexibility! Respite Providers provide relief from care-giving responsibilities for families by engaging the child in fun recreational activities that support the interest and abilities of the child. Respite services typically occur after school hours including evenings and weekends. In general, services should be provided in a way that most families can replicate regardless of socioeconomic background.
