Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service. The programs of Community Care Network serve more than 3,000 people each year throughout Rutland County. The dedicated 350 plus employees of CCN are focused on improving the health and well-being of Rutland County residents and their families living with mental illness, developmental disabilities and substance use disorders.
About The Role:
Within this team oriented setting, we have a full time, Bachelor's Level, School Based Case Manager position available. The Case Manager will provide direct clinical services such as: supportive counseling, academic and community support as needed to ensure the integration of educational and therapeutic services for students in a classroom environment. The Case Manager will support and enhance the emotional wellbeing of students and their families.
